Student-Athlete Code of Conduct
CVS Student-Athlete Code of Conduct.pdfThis document sets out the Athletic Mission Statement for Chippewa Valley Schools, as well as Athletic Program Beliefs, Eligibility, Rules, Transportation Policy, Attendance Requirements and Vacation Policy, Academic Standards for Athletes, Conduct Expectations for Athletes, Consequences and Appeal Procedures. All athletes and their parents are expected to read and to be familiar with this document. Any questions should be directed to the coach or Athletic Director. A student, by participating on any team, is voluntarily submitting to abide by the rules, requirements, standards, and regulations listed in the CVS Code of Conduct, as well as any rules, requirements, standards, and regulations of the team. Being on a team is a privilege, not a right. Therefore, it is crucial for student athletes to understand the Athletic Department’s expectations of them, and what they may expect from the athletic program.
MHSAA Phyiscal Form
MHSAA Physical Card.pdfThe MHSAA Physical Form but be completely filled out and signed before submission to the Athletic Office. Valid athletic physicals for the current school year must be dated on or after April 15th of the previous school year and signed by a MD, DO, PA, or NP.
Participation Fee Refund Request
CVS Athletic Fee Refund Request Form.pdfIf you have paid the Athletic Participation Fee for your child and would like to ask for a refund, the attached Form must be completed by the family, signed by the head coach, and then submitted to the Athletic Department for approval.

This document shares information on all of our 27 varsity programs, including varsity head coach's name and email address, season of competition, number of contests, approximate number of athletes on team, and time frame for MHSAA postseason tournaments.

Athletic Participation Fees will be available in the "Optional" Tab approximately one week after the first day of practice/tryouts each season.
The Business Department will assign the Participation Fee to your child soon after receiving each team's roster.

FamilyID provides online, paperless athletic registration which must be completed once per school year.
If your child is a multi-sport athlete and you have already registered for the current school year, you do not need to register again until next school year.
Life of an Athlete Survey

All CVS student-athletes must complete the Life of an Athlete Survey once per school year. Students may only take the survey after they have logged in with the school username and password.
